Why not use some of the momey you spend on fast food and booze to slowly kill yourself on a doctor's appointment? Ya know what, killing yourself is a better plan. Carry on.

Are there no alternatives for low income people to get care where you're at? I don't think you realize the sort of damage you're doing to your organs by 1) not taking your BP meds and 2) continuing shitty lifestyle choices that exacerbate the issue. You'll wind up in an early grave if you don't take better care of yourself, bud.

Death is neither good nor bad, it just is. That wasn't my point.
I'll be entirely indifferent to his death as tinychan is only losing a village idiot. He's nothing to any of us.
Now imagine for a moment, if you will, that Bert did take charge of his life.
Really turns it around, salvages his health, sobers up, works hard, maybe even finds a mate, goes on to do something positive like community organizing, volunteer work at a soup kitchen or a cat shelter, w/e. Anything productive with his few remaining years on this Earth that's worth a damn, something that he and his mama could be proud of.
We both know that isn't likely. In all probability, he'll cling to his delusions that somehow all his problems are entirely the fault of other people and never take charge of his own life. Forever self-condemned to bawl like a bitch from the gutter of society, but never mustering up the nerve to try and actually change things for the better. The fact is he could change things and yet elects not to.
Now as to the part you might be able to appreciate and why we should remind him of his autonomy and choice... when he's on his deathbed, which is the worse fate:
A) him dying tormented by the knowledge that he could've changed and yet refused to do so because it's hard to cultivate the nerve to try and fix things, to shoulder responsibility, despite the capacity to do so always being within his grasp.
B) him dying peacefully, warmed by the smug satisfaction from the delusion that he was always at the mercy of something else and his lot in life was somehow everyone else's fault, anyone's but his.
See, he's used to being shit on, and is happy to accept that he's powerless to change. That's why it's better to remind him of his own autonomy. His death is immaterial, as all death is, but if he doesn't use his own will to change things while knowing he could've done it all along, it'll only increase his suffering in the end.
In the unlikely event, he does use his free will for positive change, then maybe he'd fuck off from here and things will be slightly better for the poor bastard(s) that do care for him. Something like that.
Thanks for listening to my TCTalk.
TL;DR Boners.

C'mon, guys, you know he's fucking someone for pay, that makes him a sex worker, just because you disagree with the arrangement is no cause to be so judgmental simply because it's taboo where you're from.
