I've known doctors in rural areas that would horse trade or if they were feeling real generous accept stuff like homemade pies in lieu of payment for hardup or elderly patients on fixed incomes, but the closest I can come up with to what you're suggesting would be doktors who are doing volunteer work, especially in the third world. In which case they're not working for room and board exactly, but they're not working for shekels either.
