I don't know about your case, but this was new content.
The software stores a hash for every file uploaded and if it already exists, the server doesn't waste space by storing another copy of it. Since the post originally had the image attached, the hash was in the database, but when you replace or delete an image which isn't linked to any other posts, the file gets deleted (as opposed to hidden which is what actually happens when a reposted image is "deleted"). So the image gets checked and matches a hash in the db so the uploaded file is discarded, then the server deletes the old image because the only post using it is being modified with a new upload. Except that new upload is actually the same image that just got deleted, so the hash stays in the database, but the file is gone.
