Is it that good that you take so much it kills you? Pie is good. But nobody would eat so much in one sitting they would die, which leads me to wonder if anyone has ever overdosed on pie.

Because it doesn't take all that much to overdose on so it's easy to make a mistake with it.

I think there is some heroin that is cut with fentanyl that has killed junkies in new York recently. PSH almost certainly took this.

Also, when junkies have been clean for a while, its easy to misjudge dosage when you get back on it.

One of the biggest problems with street drugs is quality control. One batch may be much stronger than another (which can result to fatal dosing errors) or as someone else has already suggested they may be cut or substituted with other opioids which may much be much more potent by weight and too much, even assuming some degree of cross tolerance, it would be easy to massively overdose on say butyl-fentanyl when you're used to heroin.
