Now that the 2 nonces Namefag and Nicholas are no longer around to protect him, only Fuckalms remains. And although The Doctor has gone into over-drive in sucking his cock, there will come a day when Fuckalms grows out of this place and moves on, leaving Doc completely on his own, without anyone to protect him from the bullying.
